Each attendee must register individually.
Assumption of Risk: The registered participant assumes all risks and waives any claim of liability against Southwest Seattle Historical Society (SWSHS) and its employees and volunteers for any personal injury or property damage sustained by me during or as a result of my participation in an event sponsored by SWSHS. The undersigned is aware that events have risks, including physical exertion, traffic, and weather. The undersigned agrees that SWSHS representatives may arrange, at my cost, for medical treatment and emergency evacuation as they deem necessary for my health and safety.
Media Release: The registered participant grants permission to SWSHS, hereinafter known as the “Media” to use my image (photographs and/or video) for use in Media publications including but not limited to videos, email blasts, recruiting brochures, newsletters, magazines, general publications, websites and/or affiliates. The undersigned hereby waives any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the image.
The undersigned agrees to all of the above for themselves and on behalf of all family members and minors for whom they are responsible.
All participants are required to check in and agree to the above waiver.
